The Clinical Excellence Commission’s (CEC’s) Quality Audit Reporting System (QARS) is an electronic tool for preparing and conducting audits and surveys.
QARS is available for use by local health districts, speciality networks and NSW health service providers (Health Entities) to collect data that will help to drive the implementation of safety and quality systems and improve the quality of the health care they provide. This may include measuring existing and emerging healthcare issues, including governance and assurance and patient safety. An audit or survey can also be used by a Health Entity to meet their accreditation requirements as determined by the National Safety and Quality Health Service Standards of the Australian Commission on Safety and Quality in Health Care.
Quality and legality of survey content
The CEC is not responsible in any way for the quality or legality of the content of audits or surveys prepared or conducted using QARS. The Health Entity that has used QARS to prepare and provide this audit/survey to you is responsible for ensuring that the content of the audit/survey is prepared in compliance with state and federal legislation and NSW Health policies and guidelines including NSW Health’s Health Information Privacy Leaflet for Patients which is available on the NSW Health website.
Survey participation and collection of survey response information
Participation in audits or surveys is voluntary. By responding to an audit or survey, you are consenting to the collection of the information you provide in response which may include your personal information. This information will be retained for security and quality purposes.
Other than personal and/or health information, all information collected in response to an audit/survey is owned by the Health Entity conducting the audit or survey. The Health Entity is responsible for any analysis of data, and actions, decisions or outcomes arising from the data. The Health Entity is also responsible for letting you know how it will use information provided in response to a survey.
Privacy and personal information
All requests for access to personal and/or health information, privacy complaints or enquiries about privacy should be addressed either to the co-ordinator of the audit/survey, the manager of the Health Entity or the privacy contact officer of the Health Entity that has sent you this audit/survey.
Please do not forward an audit/survey without the permission of the Health Entity that sent it to you.

The CEC will follow state and federal legislation and NSW Health policies regarding the secure storage of survey response information, in order to protect survey response information from unauthorised access, loss or other misuse.
3. Use or disclosure of information
The CEC will not use or disclose audit/survey data to QARS unauthorised persons, except where required by law.
4. Limitation of liability
The QARS survey module software is provided on an 'as is' and 'as available' basis. Neither CEC (as the designer of QARS) nor NSW Health warrant that an audit/survey or E-Mail sent from QARS will be free of viruses or other harmful components CEC and NSW Health will not be responsible or liable:
